Amir is a 44-year-old carpenter who presents with pain in his right buttock and posterior thigh. He reports that the pain began three weeks ago, immediately following a fall from a ladder, during which he landed on his right buttock.

He describes his posterior thigh pain as mild, with a burning quality and occasional tingling. His buttock pain and posterior thigh symptoms become worse when he sits for prolonged periods and applies pressure to his right buttock. He reports that hip movements, such as an abrupt change of direction when walking, aggravate his pain. He feels relief by limiting his activities and applying ice to the buttock region. He does not have any lower back pain or stiffness.

- Adaptation and Compensation – The body may adapt to accommodate the underlying injury or condition, resulting in changes in pain perception. This adaptation process can lead to a shift from an initial sharp and intense pain to a more tolerable but constant ache.

Importantly, he has never previously experienced right buttock pain or pain and tingling in his right posterior thigh before this current episode.
